Waunakee woman faces two homicide charges for Monroe County fatal crash on I-94

SPARTA, Wis. (WKBT) — A 41-year-old Waunakee, Wis., woman has been charged with two homicide counts in connection to a four-vehicle, wrong-way crash on I-94 near Warrens in which one person died and two others were injured seriously.

Carrie Herbst was charged Wednesday with first-degree reckless homicide and homicide by intoxicated use of a motor vehicle in connection with the Oct. 3 crash on I-94.

Herbst was eastbound in a minivan on the interstate around 9 p.m. when she exited near Three Bears Resort, according to the criminal complaint from the Monroe County Sheriff’s Office.

Herbst then turned around, traveled up the exit ramp, and traveled west in the eastbound lanes for about 1 mile and collided with other vehicles, according to the complaint.

A witness told law enforcement he had traveled behind Herbst’s vehicle on the interstate and exited at the same time, according to the complaint. He reported that he observed her vehicle weave across lanes.

Two people were injured seriously in the crash and another individual died from his injuries.

Following an investigation, Monroe County sheriff’s deputies arrested Herbst, who made her first court appearance in Monroe County Circuit Wednesday. Her bond was set at $15,000, which she posted, according to court records.

She is due back in court on Jan. 10.

The Monroe sheriff’s office asks that anyone who may have been a motorist on the interstate at the time of the crash or anyone who has information about it to contact to contact that office or the Wisconsin State Patrol.
